Southwest Behavioral & Health Services seeks a Case Manager to provide in-home, community, or office-based support to adults and families, including crisis stabilization. You will assess needs, develop service plans, coordinate with care teams, and help clients access community resources, employment, and daily living skills.

Qualifications: Bachelor's degree in Social Work or related field, 1 year of behavioral health experience, valid AZ driver's license; fingerprint clearance may be required.

• Minimum 1 year of full-time behavioral health experience.
• Must be at least 21 years of age.
• Fingerprint clearance or eligibility to receive upon hire.
• Valid Arizona driver’s license and use of personal vehicle.

• Provide comprehensive in-home and community-based support to adults and families.
• Assess consumers according to policies and clinical protocols and develop service plans.
• Coordinate with care teams and review interventions to adjust plans of service.
• Monitor mental status and response to treatment and update plans as needed.
• Provide crisis intervention and connect consumers with community resources.
