Qualifications

• Bachelor’s degree in the behavioral health field plus one year of relevant experience in behavioral health requested.
• Combination of experience and education totaling 2 years acceptable.

Responsibilities

• Intake screenings (Completing intake paperwork and referrals)
• Completing annual behavioral health assessments, including service planning and support planning
• Facilitating CFTs and ARTs
• Identifying gaps in care for our members
• Completing SMI and SED evaluation
