Job summary

White Oak Medical Center in Silver Spring, MD seeks a Care Navigator Concierge to support our Outpatient Care Navigation team. You will schedule appointments, coordinate follow-ups, and ensure timely documentation to aid patients and families post-discharge.

Requires 1–2 years in a medical setting and a current BLS certification. Work hours are 40 per week, day shift, with a competitive benefits package. Strong customer service and knowledge of local physician practices are essential.

Qualifications

• High school diploma required.
• 1-2 years' experience in a medical setting.
• Active American Heart Association BLS certification required.

Responsibilities

• Schedule appointments for patients and families according to discharge instructions.
• Maintain awareness of local physician practices for scheduling follow-ups.
• Notify manager if challenges arise in booking follow-up appointments.
• Notify team members when patients require additional support post-discharge.
• Complete required documentation in a timely and accurate manner.
