Job summary

Griffin Hospital is seeking an Outpatient Navigator to coordinate post-ED follow-up for recently discharged patients, bridging ED, primary care, specialists, and community resources to improve continuity and outcomes.

The role requires MA or LPN experience, strong communication, and EMR proficiency to schedule follow-ups, address barriers, and document outreach in the EMR, supporting reduced readmissions and patient satisfaction.

Qualifications

• Current certification/licensure as MA or LPN.
• Minimum of 2 years of clinical healthcare experience.
• Experience in ED, ambulatory care, care coordination or population health preferred.

Responsibilities

• Review ED discharge reports and identify patients needing follow-up.
• Contact discharged patients to assess needs and coordinate timely follow-up.
• Schedule appointments with PCPs, specialists, imaging, labs, or other services.
• Assist patients with barriers to care including transportation and insurance concerns.
• Educate patients on treatment plans, medications, and warning signs.
• Document outreach and follow-up outcomes in the EMR.
