Job summary

Adventist HealthCare in Silver Spring, MD seeks a Services Assistant for the Post Acute Care Services Department to facilitate referrals and ensure smooth transitions to post-acute care. You will work with hospital care navigation teams to process authorizations and coordinate patient moves to SNF or home care.

Expected 40 hours weekly on days, with occasional weekend coverage. High school diploma required; a AA in Business or Healthcare is preferred; 3-5 years in post-acute settings is

Qualifications

• High school diploma or equivalent required.
• AA Degree in Business or HealthCare preferred.
• 3-5 years Home Care, Acute Rehab, Sub-Acute Rehab, or other Post-Acute setting preferred.

Responsibilities

• The PAC Assistant processes referrals quickly and effectively through admission workflows using EMR and internal systems.
• Supports Care Navigation teams with referrals, authorizations and coordination into Post-Acute Care Services.
• Monitors discharge systems and communicates with clinical and non-clinical liaisons.
• Provides coverage for PAC Intake and assists with reporting, scheduling and projects.
