Job summary

Meritus Medical Center in Hagerstown, MD is seeking a Community Navigator to support patients as they move through the healthcare system. You’ll join the outpatient care management team to improve outcomes through compassionate outreach and coordinated care.

Responsibilities include helping patients complete applications, arranging transportation, reminders, and supporting care coordination from initial contact to case closure while engaging with community resources and advocating for patients.

Qualifications

• Education: High school diploma or equivalent; Associate Degree preferred.
• Experience: Clerical experience in a healthcare setting preferred; Familiarity with Microsoft Office, fax systems, and electronic medical records preferred.
• Skills & Abilities: Strong communication and computer skills; Knowledge of community resources (preferred); Basic understanding of medical terminology; Ability to prioritize tasks, stay organized, and manage multiple responsibilities.

Responsibilities

• Assist patients with completing applications, registration forms, and accessing community resources
• Help patients connect with transportation services and provide appointment reminders
• Support seamless care coordination from initial contact through case closure
• Provide general office and clerical support to the outpatient care management team
• Help patients navigate the healthcare system with confidence and clarity
• Contribute to population health goals through outreach and patient engagement
