Meritus Medical Center in Hagerstown, MD is seeking a Wellness Coordinator to support patients in accessing preventive services and improving their wellbeing. This on-site, full-time role involves outreach, care coordination, and collaboration with physicians, nurses, and staff.

You will call patients, explain preventive services, educate on healthy choices, close care gaps, assist with care management, and help with general office tasks.

Qualificações

• Education: High school diploma or equivalent required.
• Preference for an Associate degree in a related field.
• Healthcare clerical experience is preferred.

Responsabilidades

• Be the voice of wellness: outreach calls guiding patients to preventive care like Medicare Wellness Visits.
• Educate patients on preventive care and help close care gaps affecting health outcomes.
• Collaborate with physicians, nurses, and staff for integrated patient care.
• Support quality improvement through documentation, process improvements, and staff presentations.
• Assist with general office tasks and population health initiatives.
