Baycare Health Partners Inc. seeks an Outpatient Care Coordinator to coordinate care, provide outreach and coaching, and connect patients with community resources. You will document encounters, perform health risk assessments, and support transitions of care while collaborating with the care team to improve outcomes.

The role emphasizes patient-centered coaching, proactive follow-ups, and adherence to care protocols within a primary care setting.

Qualifications

• High school diploma or GED; medical assistant program graduate.
• Credentialing must be awarded by a third-party body (AAMA/AMT/CCMA) if applicable.
• 3-5 years of experience as a Medical Assistant.
• MA Driver’s License required.
• Preferred Certification: Community Health Worker

Responsibilities

• Coordinate care for a patient population to improve outcomes.
• Provide outreach and coaching on self-care and link to community resources.
• Document encounters clearly and accurately in medical records.
• Perform health risk assessments and orient patients to the practice.
• Assist with transitions of care and follow-up after discharge.
• Screen for preventive care and disease management needs.
• Collaborate with care team to maximize patient satisfaction.
