You’ll collaborate with nurses, physicians, and care teams to coordinate services, authorize post-acute care when needed, and connect patients with community resources while ensuring regulatory compliance and efficient workflow.

Qualifications

• Bachelor's degree in social work, psychology, or healthcare field.
• HS with LPN license acceptable per criteria.
• Experience in medical social work or LPN preferred.

Responsibilities

• Collaborate with patients and care teams to optimize outcomes.
• Coordinate comprehensive transition of care and post-acute referrals.
• Identify barriers to transitions, including SDOH.
• Ensure regulatory compliance and effective length of stay management.
