Job summary

Houston Behavioral Healthcare Hospital (HBHH) is seeking a full-time Outpatient Navigator for our outpatient department, Monday through Friday, 8:00 AM to 4:00 PM.

The Navigator welcomes and supports newly admitted patients from community partners, referring hospitals, and surrounding areas, coordinating co-response initiatives, crisis intervention, referrals, and access to community resources. This role emphasizes patient-centered care and collaboration with the clinical team.

Qualifications

• Bachelor's degree required; Mental Health emphasis preferred.
• 1–5 years experience with adults, adolescents, families in addiction and mental health.
• Bilingual preferred.
• CPR and Hospital De-escalation Certification required or during Orientation.

Responsibilities

• Screen adults/adolescents to assess eligibility for Outpatient program.
• Coordinate care transition across behavioral health system and continuum of care.
• Assist with transportation coordination to/from programming.
• Actively manage and track outside and internal referrals to the Outpatient Program.
• Build and maintain a resource bank of updated online mental health resources for the care team and partners.
• Coordinate care between departments (UR, Business Office, Intake, Inpatient) for Step Down Processes.
• Communicate changes in patient status to the Outpatient Clinical Team.
• Support daily outreach and case management per the Outpatient Director.
