Job summary

Houston Behavioral Healthcare Hospital is seeking a full-time Outpatient Navigator. Mon-Fri, 8:00 AM to 4:00 PM, you will welcome and support newly admitted patients from community partners and referring hospitals, prioritizing patient-centered care within HBHH's outpatient department.

You will coordinate crisis interventions, referrals, transportation, and case management while collaborating with the Outpatient Clinical Team to improve outcomes and promote mental health wellness across the

Qualifications

• Bachelor's degree with mental health emphasis or related field preferred.
• Experience with adults, adolescents, and families in addiction/mental health settings preferred.
• CPR certification and hospital de-escalation training preferred.

Responsibilities

• Completing screenings of adults and/or adolescents to assess eligibility for Outpatient program requirements.
• Assist and support transition of patient care across levels of care within the community.
• Provide assistance coordinating transportation to and from programming.
• Actively manage and track referrals to the Outpatient Program.
• Build a resource bank of online mental health resources for HBHH staff and community.
• Coordinate care between departments for Step Down Processes.
• Communicates changes in patients status with the Outpatient Clinical Team.
• Maintains patient outreach and daily activity updates for assigned cases.
• Adhere to HBHH policies, HIPAA, Trauma-Informed Care and Patient Safety standards.
• Develop partnerships with community mental health entities for inter-agency communication.
