Coastal Horizons Center, Inc. seeks an Intake/Access Client Care Navigator to serve as the primary clinical access point for adults seeking walk-in behavioral health services.

This role supports Adult Outpatient Services and Behavioral Health Urgent Care (BHUC) by performing brief screenings and routing to the appropriate initial service. The navigator works with licensed clinicians and community partners, ensuring trauma-informed care, confidentiality, and timely handoffs while coordinating

Qualifications

• Must meet North Carolina requirements for Qualified Professional designation.
• Minimum of one year in behavioral health access, crisis services, or related field.
• Knowledge of co-occurring conditions and escalation procedures.
• Ability to engage adults with emotional distress using trauma-informed approaches.
• Proficiency with EHRs and standard office software.

Responsibilities

• Provide welcoming, trauma-informed initial contact to adults seeking walk-in behavioral health services.
• Complete standardized screenings addressing concerns, symptoms, substance use, safety risks, and needs.
• Determine preliminary urgency using approved tools and routes.
• Route individuals to Adult Outpatient Services or BHUC as appropriate.
• Prioritize urgent cases for BHUC assessment and intervention.
• Coordinate access to routine assessment or referrals when BHUC is not indicated.
• Consult with licensed clinicians or program leaders when unclear.
• Identify and escalate suicide or violence risk and initiate emergency procedures when needed.
• Document findings and service dispositions accurately in the EHR.
• Maintain confidentiality and follow documentation requirements.
• Collaborate with BHUC staff, nursing, prescribers, EMS, and community partners.
