Job summary

OLE Health is hiring a BH Navigator for the Integrated Behavioral Health team in California. The role focuses on closing referral loops, supporting access to behavioral health care, and acting as a key resource for patients and families.

Responsibilities include coordinating internal and external services, maintaining EMR documentation, and supporting care team collaboration to improve health outcomes. BLS certification is required.

Qualifications

• Bachelor’s level or equivalent in social services preferred.
• Experience collaborating with care teams and external partners.
• Strong communication and customer service skills required.
• BLS certification required.

Responsibilities

• Process and close the loop on internal and external behavioral health referrals.
• Support patient access to behavioral health care and real-time screening for virtual appointments.
• Serve as point of contact, advocate, and informational resource for patients and families.
• Facilitate access to internal and external services and coordinate care.
• Identify and address gaps in care and coordinate with care teams.
• Document timely and efficiently in the EMR and participate in team meetings.
