Job summary

University Hospitals is seeking a Patient Navigator for Behavioral Health to support population management within the Behavioral Health Ambulatory Outpatient Services. This role emphasizes clear communication with patients, providers, and UHACO staff to coordinate care and connect individuals to available resources.

You will analyze data to identify at-risk patients, conduct remote outreach, schedule appointments, educate patients and families, and document progress in the electronic records.

Qualifications

• Bachelor's Degree in psychology or related field (Required).
• 2+ years in health care setting (Required).
• Experience with multiple healthcare data sources including Electronic Medical Records, registration and scheduling systems (Required).
• Experience facilitating telephonic outreach (Preferred).

Responsibilities

• Identify at-risk patients and help develop specific care plans to address health concerns and gaps in care.
• Provide patient education and connect patients with resources within UHACO, UH, and the community.
• Schedule behavioral health appointments and document outreach using required systems/workflows.
• Educate patients and families to make informed Behavioral Health care decisions and provide culturally competent support.
• Coordinate referrals between plan members, providers, and UHACO staff.
• Support ongoing outreach to monitor patient status and progress through care plans.
