Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, Nebraska. Flexible scheduling includes full-time, part-time, or PRN positions with a $5k sign-on bonus.

Responsibilities include planning, directing OT services, and coordinating with the interdisciplinary team to maintain high-quality resident care while ensuring proper documentation and compliance with CMS standards.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention per scope of practice.
• Implement treatment plans and monitor goals per plan of care.
• Communicate evaluation findings and treatment plans to residents and families.
• Schedule therapy sessions with team cooperation.
• Document and bill occupational therapy services per plan.
• Perform other duties to effectively treat patients.
• Comply with CMS documentation standards daily.
