Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, NE. This full-time opportunity includes a $5,000 sign-on or relocation bonus and flexible options including part-time or PRN.

The OTA will plan, develop, and direct occupational therapy services, conduct screenings, implement treatment plans, and collaborate with an interdisciplinary team to ensure high-quality resident care.

Qualifications

• Associate's degree required in Occupational Therapy.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention per scope of practice.
• Develop and follow treatment plans with short- and long-term goals.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families, and the care team.
• Establish individual resident therapy schedules with team members.
• Document and bill occupational therapy services according to plan of care.
