Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, NE. Flexible Full Time, Part Time, or PRN positions available, with a $5,000 sign-on or relocation bonus and flexible scheduling.

Responsibilities include planning, organizing, developing, and directing OT services, screening residents, implementing treatment plans, documenting care, and collaborating with the interdisciplinary team to ensure high-quality

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Perform treatment plans in line with established goals.
• Interpret and communicate evaluation findings to residents and families.
• Establish resident therapy schedules with team members.
• Conduct therapy and ensure proper documentation and billing of services.
• Perform other duties to effectively treat patients.
• Comply with CMS documentation standards daily.
