Job summary

Evolve Therapy Services, LLC is seeking an Occupational Therapist to lead in-house therapy at a Skilled Nursing Facility in North Platte, NE. The role supports comprehensive resident care and requires active licensure in the state(s).

Full-time position with options for Part Time or PRN and flexible scheduling. A $10,000 sign-on bonus is available, with opportunities for growth within the organization.

Qualifications

• Degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screenings of residents to determine the best intervention or treatment.
• Evaluate residents to inform treatment plans and set goals.
• Communicate evaluation findings and plans to residents, families, and team members.
• Establish individual therapy schedules with the team.
• Deliver therapy and document/bill OT services.
• Perform other duties as needed to treat patients.
• Oversee COTA and ensure documentation and reimbursement practices are followed.
