Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility partner in North Platte, NE. Full Time, Part Time, or PRN positions available with flexible scheduling and a $5,000 sign-on or relocation bonus.

You will screen residents, implement treatment plans, document and bill for services, and coordinate with the interdisciplinary team to maintain high-quality resident care per CMS standards.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best intervention based on scope of practice.
• Perform treatment plans and follow short- and long-term goals.
• Communicate evaluation findings and plans to residents and care teams.
• Establish therapy schedules with team members.
• Document and bill therapy services according to plan.
• Ensure CMS documentation standards daily.
