Job summary

Continuum Therapy Partners, LLC is seeking a skilled Occupational Therapist for PRN daytime shifts at Claymont Health & Rehabilitation in Uhrichsville, Ohio. You will provide care with the same clinical standards as the full-time team and help maintain continuity of care.

The role requires an OT degree, state license, NBCOT, and preferred 1 year experience. You will evaluate patients, develop treatment plans, document progress, and educate families about adaptive equipment and home safety.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
