Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist for a PRN on-site role at Montgomery Nursing & Rehab Center in Hillsboro, IL. You will provide high-quality OT services, assess needs, and collaborate with a leadership team to ensure consistent care.

The position emphasizes flexible scheduling, patient-centered plans, and education for patients and families on adaptive equipment and home safety. Licensure and NBCOT certification are required.

Qualifications

• Master's or Doctorate in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
