Job summary

Continuum Therapy Partners is seeking an Occupational Therapist for PRN daytime coverage at Cornell Health Services in Cornell, WI. The therapist will deliver high-quality care, maintain clinical standards, and support continuity of care within our team.

Responsibilities include evaluating and developing individualized treatment plans, documenting progress, and educating patients and families on adaptive equipment and home safety measures. A state license and NBCOT are required.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy
• Current, unencumbered state OT license; NBCOT certification
• 1 year of OT experience preferred

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required

• Current, unencumbered state Occupational Therapy License; NBCOT Certification required

• 1 year of previous experience as an Occupational Therapist preferred

• Evaluates and develops individualized treatment plans to improve patients' functional independence

• Documents patient progress and modifies treatment plans based on clinical outcomes

• Educates patients and families on adaptive equipment and home safety recommendations
