Job summary

Continuum Therapy Partners is seeking a PRN Occupational Therapist to join our Altoona, WI team. You will evaluate and treat patients, develop individualized plans, educate families on equipment and home safety, and document progress while adjusting plans based on outcomes.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required

• Current, unencumbered state Occupational Therapy License; NBCOT Certification required

• 1 year of previous experience as an Occupational Therapist preferred

• Evaluates and develops individualized treatment plans to improve patients' functional independence

• Documents patient progress and modifies treatment plans based on clinical outcomes

• Educates patients and families on adaptive equipment and home safety recommendations
