Job summary

AED CONSTRUCTORES in San Rafael, CA, seeks an Occupational Therapist to evaluate and treat patients in a Skilled Nursing Facility, developing individualized plans to support independence.

The role requires California OT licensure, BLS/CPR, and 1+ year of SNF/geriatric OT experience. You will collaborate with a multidisciplinary team, document care, and participate in CE opportunities to advance patient outcomes.

Qualifications

• Active California OT license required.
• Current BLS/CPR certification.
• Minimum 1 year OT experience, preferably in SNF or geriatric settings.
• Strong teamwork and patient-care skills.
• Willingness to participate in CE.

Responsibilities

• Evaluate and treat patients with physical, cognitive, and functional limitations in a Skilled Nursing Facility setting.
• Develop and implement individualized occupational therapy treatment plans.
• Train patients in activities of daily living (ADLs) such as dressing, bathing, and feeding.
• Recommend adaptive equipment and environmental modifications to improve independence.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and compliant documentation of patient care and outcomes.
