Job summary

AED CONSTRUCTORES in Sacramento, CA welcomes an Occupational Therapist to join a Skilled Nursing Facility team. The OT will work with geriatric patients to restore independence through individualized treatment plans and ADL training in a collaborative, patient-centered environment.

Candidate should hold a California OT license and current BLS/CPR. SNF or geriatric experience is preferred, with a focus on compassionate care and measurable outcomes for quality of life improvement.

Qualifications

• Licensed Occupational Therapist in California (CA)
• BLS/CPR certification current
• Minimum 1 year OT experience preferred (SNF/geriatric)
• Strong problem-solving and patient-care skills
• Ability to collaborate in a multidisciplinary team

Responsibilities

• Evaluate and treat patients with physical, cognitive, and functional limits in SNF
• Develop and implement individualized OT treatment plans
• Train patients in activities of daily living (ADLs)
• Recommend adaptive equipment and environmental modifications
• Monitor progress and adjust plans accordingly
• Maintain accurate documentation of patient care and outcomes
