Job summary
AED CONSTRUCTORES in Modesto, CA is seeking a skilled Occupational Therapist to support a Skilled Nursing Facility. You will assess and treat geriatric patients, helping them regain independence in daily activities and rehabilitation after illness or surgery.

Qualifications
- Active state license as an Occupational Therapist in California.
- Current BLS/CPR certification.
- Minimum 1 year of OT experience, preferably in SNF or geriatric settings.
Responsibilities
- Evaluate and treat patients with physical, cognitive, and functional limitations in a SNF.
- Develop and implement individualized OT treatment plans.
- Train patients in activities of daily living (ADLs) such as dressing, bathing, and feeding.
- Recommend adaptive equipment and environmental modifications to promote independence.
- Monitor patient progress and adjust treatment plans as needed.
- Maintain accurate documentation of patient care and outcomes.
