Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Olivia, MN. The OTA will plan, organize, develop, and direct Occupational Therapy Services in the facility in accordance with federal, state, and local standards, ensuring the highest quality resident care at all times.

The role supports regular screenings, treatment planning, and collaboration with the interdisciplinary team.

Qualifications

• Associate's degree in Occupational Therapy.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention per scope of practice.
• Perform treatment plans and monitor progress toward goals.
• Communicate evaluation findings and recommendations to residents, families, and team members.
• Establish resident therapy schedules with the team.
• Document and bill occupational therapy services according to plan of care.
• Comply with CMS documentation standards daily.
• Perform additional duties as needed to effectively treat patients.
