Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Olivia, MN. The role requires an OTA licensure and an Associate's degree in Occupational Therapy, with full-time, part-time, or PRN options along with a $5,000 sign-on and tuition reimbursement.

Responsibilities

• Plan, organize, develop, and direct Occupational Therapy Services in the facility.
• Screen residents to determine best intervention per scope of practice.
• Perform treatment plans and follow short- and long-term goals with supervising therapist.
• Interpret evaluation findings and communicate with residents, families, and the interdisciplinary team.
• Establish resident therapy schedules in coordination with team members.
• Document and bill occupational therapy services according to plans and CMS standards.
• Comply with CMS documentation standards on a daily basis.
