Job summary

Evolve Therapy Services is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Olivia, MN. You will plan and deliver occupational therapy services under supervision to support resident care.

The ideal candidate has an Associate's degree in Occupational Therapy, active OTA licensure, and a commitment to ongoing professional development. Full-time with a $5,000 sign-on bonus and tuition reimbursement; part-time/PRN options available.

Qualifications

• Associate's degree in Occupational Therapy is required.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine appropriate intervention or treatment.
• Perform treatment plans and follow short- and long-term goals.
• Communicate evaluation findings and plans to residents and teams.
• Establish resident therapy schedules with the team.
• Conduct therapy according to treatment plan and document/bill services.
• Perform other duties to support patient care.
• Comply with CMS documentation standards daily.
