Job summary

Evolve Therapy Services is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Olivia, MN. The role involves planning, directing, and delivering occupational therapy services under supervision to ensure resident care meets standards.

Full-time position with a $5,000 sign-on and tuition reimbursement; part-time and PRN options available. Requirements include an Associate's degree and active OTA licensure; ongoing CEUs are encouraged.

Qualifications

• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Conduct regular screening of residents to determine best intervention for each patient.
• Develop and implement treatment plans aligned with goals.
• Interpret and communicate evaluation findings and recommendations to residents and families.
• Schedule therapy sessions in coordination with the care team.
• Document therapy services and ensure billing compliance.
• Perform other duties to effectively treat patients.
• Maintain CMS documentation standards daily
