Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Olivia, MN. This full-time position offers a $5,000 sign-on bonus and tuition reimbursement, with part-time and PRN options available.

Responsibilities include conducting screenings, implementing treatment plans, collaborating with an interdisciplinary team, and ensuring proper documentation and billing while maintaining CMS standards.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular resident screenings to determine best intervention per scope of practice.
• Perform treatment plans and follow short- and long-term goals as directed by supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents and families.
• Establish individual resident therapy schedules with team members.
• Document and bill OT services in accordance with plan of care.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily.

Education

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice
