Concept Rehab seeks a licensed Certified Occupational Therapy Assistant to work under the supervision of the Licensed Occupational Therapist. You will implement the treatment plan, monitor patient progress, and collaborate with the care team to support mobility, cognition, and daily living activities.

You will document services, participate in team meetings, provide in-service training, and help discharge planning.

Qualificações

• Licensed in the state(s) where services are being provided as a Certified Occupational Therapy Assistant.

Responsabilidades

• Treat patients under the supervision of the Licensed Occupational Therapist according to the plan of care.
• Communicate patient progress to the Occupational Therapist, patient and family, and facility staff.
• Participate in Plan of Care Meetings and Interdisciplinary Team Meetings.
• Record treatment notes and progress notes; provide input for discharge summaries and billing per policy.
• Instruct patients’ families and nursing staff in maintenance programs under supervision.
• Participate in discharge planning under supervision of the Occupational Therapist.
• Maintain professional documentation of services in the medical record.
