Concept Rehab is seeking a licensed Occupational Therapy Assistant to provide patient care under the supervision of a Licensed Occupational Therapist. The OTA will document services, participate in care planning meetings, and train families and staff on maintenance programs.

Reports to the Therapy Program Manager and collaborates with physicians and the health team to deliver high-quality rehabilitative care.

Qualifications

• Licensed in the state(s) where services are provided as a Certified Occupational Therapy Assistant.
• Record and document treatment notes, progress notes, and discharge summaries.
• Participate in Plan of Care Meetings and Interdisciplinary Team Meetings.

Responsabilités

• Treat patients under the supervision of the Licensed Occupational Therapist.
• Maintain professional documentation of services in the medical record.
• Instruct patient families and/or nursing staff in maintenance programs under supervision of the Occupational Therapist.
• Participate in discharge planning under the supervision of the Occupational Therapist.
