Brightpoint in Indiana is seeking a case management professional to guide youth toward self-sufficiency. You will lead one-on-one assessments, develop individualized work plans, and monitor progress through various visits and follow-ups.

The role requires engaging with referrals, maintaining data records, and supporting youth in pursuing HSE completion, employment, and further education. The ideal candidate has a degree in psychology or social work or equivalent experience, plus strong

Competenze

• Bachelor’s Degree or a minimum of 3 years social service case management experience required.
• Public Affairs, Social Work, or Psychology preferred.
• Experience working with youth accompanied by knowledge of adolescent development issues.
• When driving for Brightpoint, must have a valid Indiana driver’s license and maintain minimum liability limits of $100,000/$300,000 on auto insurance.

Mansioni

• Provide one-on-one case management and initial client assessment.
• With youth participation, devise individual work plans to move youth to self-sufficiency.
• Monitor youth adherence through home, office, and off-site visits.
• Recruit youth by obtaining referrals from in-house and outside resources and the community.
• Record information daily in the WIOA data management system; maintain filing system.
• Assist youth enroll in High School Equivalency (HSE) classes.
• Assist with employment processes and post-secondary education opportunities.
• Advocate for the program and build community support resources.
• Communicate progress to Program Managers and address needs and problems.
• Implement Brightpoint Values at all times.
