Job summary

Mercy Clinic Orthopedics Southfork is seeking a compassionate Registered Nurse (RN) Nurse Navigator to guide patients through their orthopedic care journey, from initial consultation to recovery. You will coordinate care, provide education, and support patients and families.

Working with surgeons, APPs, therapists, and care teams, you will ensure timely, high-quality care and an exceptional patient experience, including discharge planning and post-operative follow-up.

Qualifications

• RN license or state registration required.
• 2–3 years nursing experience in healthcare.
• BLS certification required.

Responsibilities

• Coordinate and navigate care for orthopedic patients throughout evaluation, treatment, surgery, and recovery.
• Educate patients and families about diagnoses, procedures, and post-operative care.
• Facilitate communication between patients, providers, and care teams for seamless coordination.
• Develop individualized care plans with physicians and interdisciplinary teams.
• Support pre-operative planning, discharge coordination, and post-operative follow-up.
• Monitor patient progress and address barriers to care.
• Maintain accurate documentation in EPIC and other systems.
• Track outcomes and participate in quality improvement initiatives.

Skills

2-3 years nursing experience
