Job summary

Mid-State Health Center is seeking an Orthopedic Patient Care Coordinator to serve as a primary liaison among orthopedic providers, patients, families, and clinical staff in Plymouth, NH. You will schedule appointments, coordinate imaging and referrals, and support the care team with documentation in the EHR.

The role requires a High School Diploma or GED; medical office/orthopedic experience is preferred, with strong communication and organizational skills.

Qualifications

• High School Diploma or GED required.
• Experience in a medical office or orthopedic setting preferred.
• Knowledge of Electronic Health Records and healthcare information systems.

Responsibilities

• Provide direct clinical support to orthopedic clinicians and staff.
• Coordinate orthopedic appointments, imaging, referrals, and authorizations.
• Communicate scheduling information and care coordination needs to patients.
• Maintain accurate documentation in the EHR and ensure readiness for visits.
