Job summary
Holyoke Medical Center is seeking a Nurse Navigator to coordinate patient care before surgery, during recovery, and after discharge. You will work closely with providers, patients, and families to ensure timely, high-quality care in a collaborative Orthopedics & Sports Center setting.
The role requires an RN license in Massachusetts, a BSN preferred, and 3–5 years of clinical experience with strong communication and organizational skills. Competitive salary and benefits are offered.
Qualifications
- RN license in Massachusetts
- Bachelor's degree in Nursing preferred
- 3–5 years of clinical experience, case management preferred
- Strong written and verbal communication and interpersonal skills
Responsibilities
- Coordinate pre-procedure evaluations, testing, appointments, and clearances.
- Educate patients on procedures, treatment plans, and the continuum of care.
- Develop individualized care plans and identify potential risks or barriers.
- Serve as liaison among patients, families, providers, and care teams.
- Monitor adherence to care processes and treatment plans.
- Coordinate discharge planning and post-discharge services.
