Job summary

Gift of Hope Organ & Tissue Donor Network in Chicago is seeking a dedicated professional to manage daily referral activity for potential organ donors. You will triage referrals, gather clinical information, assess medical suitability, and coordinate with hospital teams to ensure timely evaluation and optimal coverage.

This 12-hour day shift includes alternating weekends. You will collaborate with families, ME/Coroner, and transplant centers, document findings precisely, and support staff

Responsibilities

• Triage organ referrals, obtain clinical information to determine medical suitability, provide clinical assessment to determine organ viability.
• Create case plan for organ referrals including further needs once onsite staff involved.
• Support staff through the referral process and provide additional education and information surrounding the donation process as needed.
• Coordinate onsite staff for organ donors and referral activity.
• Ensure accurate, clear clinical documentation according to current policies and procedures; communicate essential clinical information to Gift of Hope staff, transplant centers, and administrators.
• Facilitate necessary testing and services, at the direction of the AOC/Medical Director.
• Provide all medically relevant information to ME/Coroner to obtain release for donation.
• Communicate effectively with families of potential organ and tissue donors to provide information and support.
• Obtain and document authorization of organ and tissue donation in accordance with Gift of Hope policy.
• Perform physical assessment of potential donors.
• Coordinate medical management of DCD donors with the hospital team to ensure appropriate hemodynamic status.
• Participate in department quality assurance initiatives, meetings, educational activities and required training.
