Job summary

The Ko-Kwel Wellness Center OTP Care Coordinator at The-Coquille-Indian-Tribe provides treatment services for clients with substance use disorders and co-occurring mental health conditions under clinical supervision. Qualifications

• Certification as a Behavioral Health Aide, CADC, Recovery Mentor, or QMHA is required.
• One year of behavioral health or SUD case management experience is required.
• Experience organizing group and recreational activities is required.
• Healthcare setting experience with Native American patients is preferred.
• Ability to prioritize workload and meet deadlines.

Responsibilities

• Collect and compile information for assessing client functioning and goals.
• Provide skill training for patients and clients in individual and group settings to build health, budgeting, time management, organization, job skills, stress management, assertiveness, conflict resolution, vocational goals, or other patient-identified goals.
• Act as liaison with Ko-Kwel Wellness Center clinics and other community agencies.
• Provide case management services based on the individualized treatment plan and connect patients to internal and external resources.
• Assist individuals with identifying, applying for, and maintaining eligibility for public benefits as needed.
• Collect and compile information to assist patients in obtaining housing and/or employment as identified in the service plan.
• Coordinate care with the treatment team, employers, job coaches, advocates and others.
• Advocate for individuals to obtain needed resources.
• Coordinate with community housing agencies to access housing resources.
• Pursue continuing education to stay current with best practice standards.
• Provide follow-up to patients in person or by phone.
• Participate in outreach to engage participation in a non-coercive manner.
• Provide educational information about available services and how to access them.
• Maintain client documentation per standards and submit billable services through the EHR.
• Demonstrate communication and conflict management skills.
• Attend care team meetings and related sessions.
