Qualifications

• Certification as one of the listed credentials is required.
• At least one year of related behavioral health or SUD case management experience.
• Healthcare setting experience with Native American patients preferred.
• Experience organizing group and recreational activities.
• Ability to prioritize workload, multitask and meet deadlines.
• Proficiency in Microsoft Office and EHR systems.
• Valid Oregon driver’s license in good standing.

Responsibilities

• Collect and compile information for assessing client functioning and goals.
• Provide skill training for patients and clients to build health, functioning, budgeting, time management and job skills.
• Act as liaison with KWC clinics and community agencies.
• Provide case management and connect patients to resources and continuity of care.
• Assist individuals with public benefits eligibility when necessary.
• Assist housing and employment identification and applications.
• Coordinate care with the KWC treatment team, employers, advocates and others.
• Advocate for individuals to obtain needed resources.
• Coordinate with housing agencies to support housing resources.
• Pursue ongoing continuing education to stay current with best practice standards.
• Provide follow-up to patients and clients in person or by phone.
• Participate in outreach activities to engage participation non-coercively.
• Provide educational information about available services.
• Maintain all client documentation and billable services in the EHR.
• Demonstrate communication and conflict management skills.
• Attend care team meetings and other relevant meetings.
