Job summary

The Coquille Indian Tribe is seeking a Population Health Nurse Supervisor to lead nurse case management and coordinate care across Ko-Kwel Wellness Center locations. You will supervise case managers, drive program SOPs, ensure regulatory compliance, and optimize documentation and reimbursement workflows.

The role emphasizes population health, chronic disease management, and collaboration with multidisciplinary teams to improve outcomes and cost-effective care for tribal patients.

Qualifications

• RN license in Oregon is required.
• BSN preferred; ADN acceptable with experience.
• Minimum 2 years supervisory experience.
• Minimum 3 years in quality improvement, population health, case management or utilization management.
• Case Management certification or ability to obtain within 6 months of hire.

Responsibilities

• Provide leadership, supervision and performance management for nurse case managers and staff.
• Develop, implement and evaluate the Nurse Case Management Program and SOPs.
• Oversee documentation, coding, billing, and insurance reimbursement workflows.
