Job summary

Baylor Scott & White Health – Temple is seeking a Transfer Coordinator RN for an onsite PRN role in the Transfer Center. You will facilitate patient transfers, coordinate with physicians and nurses, and ensure timely, accurate information exchange to support smooth hospital stays.

Responsibilities include providing excellent customer service to internal and external partners, following protocols, and maintaining documentation.

Qualifications

• Associate's degree required.
• Minimum of 3 years of work experience.
• Valid RN license required.
• Transfer Center/healthcare coordination experience preferred.

Responsibilities

• Coordinate patient transfers with physicians, nurses and departments to promote timely and safe moves.
• Provide excellent customer service to internal and external partners.
• Gather and document information accurately from referring physicians and other institutions.
• Maintain clear, concise documentation meeting standards and required forms.
• Support Transfer Center Room Control Coordinator and supervise transfer workflows.
• Drive satisfaction among referring physicians and help expand BSWH market share.
