Job summary

Transfer Coordinator RN position at Baylor Scott & White Health- Temple, TX. Onsite night shift (7pm-7am) with rotating weekends. You will coordinate patient transfers, liaise with physicians, nurses and departments to facilitate access to facilities and providers.

Responsibilities include delivering excellent customer service, gathering information from external institutions, and maintaining clear documentation while supporting Transfer Center operations and market share growth.

Qualifications

• Associate's degree in a relevant field.
• A minimum of three years of work experience.
• Possessing a valid and current RN license is mandatory.

Responsibilities

• Ensure smooth patient transfers by working with physicians, nurses and other departments.
• Provide excellent customer service within and outside the organization.
• Gather information accurately and on time from other institutions and physicians.
• Enhance services across facilities with our information and resources.
• Ensure documentation is clear, concise and meets standards.
• Manage relationships with referring physicians and support Transfer Center operations.
