Qualifications

Responsibilities

• Advocate for patients and families, considering their needs in care planning.
• Serve as a liaison between patients, healthcare providers and stakeholders.
• Collaborate with the interdisciplinary team to coordinate care, appointments and follow-up plans.
• Assist patients in understanding and adhering to their care plans.
• Identify and connect patients with healthcare and community resources including transportation and financial assistance.
• Educate patients and families about conditions, options and self-care strategies.
• Maintain accurate documentation of interactions, plans and referrals.
