Qualifications

Responsibilities

Job Summary

This position requires the person in the role to work on-site approximately 4 times per week at up to three practices within the program's East Region (Boston, MA). The Patient Navigator is responsible for working closely with patients, families, and healthcare providers to ensure a seamless and patient-centered care experience by assisting with navigating the complexities of the healthcare system, coordinating resources, and providing support to improve patient outcomes.

• Full-time, Monday through Friday, standard business hours.
• Hybrid work model: Requires up to 4 times per week in-office within the program's East Region (Boston, MA) practice(s). Staniford Street location is possible but must be open to up to three practices.
• Remote workday requires stable, quiet, secure, compliant workstation with MGB provided equipment.
