Our Multi-Clinic Patient Navigators help guide patients through their initial visits and next steps in care, working closely with providers and clinical teams to keep the patient journey and clinic flow moving smoothly. This hands‑on role combines patient navigation, care coordination and administrative support. You may help patients navigate the hospital, coordinate follow-up needs and ancillary testing, support clinic flow, and assist with the many details that arise throughout a patient’s visit. No two days are exactly alike! We’re looking for someone who is organized, adaptable, proactive and genuinely enjoys working with patients. Responsible for working closely with patients, families, and healthcare providers to ensure a seamless and patient‑centered care experience by assisting with navigating the complexities of the healthcare system, coordinating resources, and providing support to improve patient outcomes.
Education Bachelor's Degree required. Can this role accept experience in lieu of a degree? Yes.
Experience Social work, case management or related field preferably in a clinical setting 1–2 years preferred.
