Job summary

Qualifications

• Bachelor's degree required.
• 1–2 years experience in social work, case management or related clinical field preferred.
• Strong knowledge of healthcare resources and patient advocacy.
• Excellent communication and interpersonal skills.
• Familiarity with electronic health records and case management software.

Responsibilities

• Advocate for patients and families in care planning.
• Serve as liaison between patients, healthcare providers and stakeholders.
• Coordinate patient care services, appointments and follow-up plans.
• Assist patients in understanding and adhering to care plans.
• Connect patients with community resources and support services.
• Educate patients and families about medical conditions and treatment options.
• Ensure patients are informed and empowered to participate in decisions.
