Solomon Medical Inc. is a growing post-acute care provider serving SNFs across Central and Northern California. We seek a Medical Coordinator to support our Behavioral Health team of physicians, NPs, and PAs, driving provider efficiency and compliant patient care.

You will manage patient intakes, scheduling, EMR data entry, and billing liaising, while maintaining HIPAA standards in a fast-paced, physician-led organization. Onsite opportunities in Morgan Hill and Gilroy, CA.

Competenze

• Bachelor’s degree in healthcare administration, nursing, or a related field preferred.
• Previous SNF experience or experience in a medical/healthcare setting strongly preferred.
• Familiarity with EMR systems and medical insurance processes.
• Basic understanding of patient care, medical terminology, and HIPAA requirements.

Mansioni

• Obtain daily nursing reports regarding acute patients and prescription refill needs.
• Manage and schedule provider visits for physicians and APPs in accordance with Medicare requirements and Solomon Medical guidelines.
• Enter new patient demographics and insurance information into the EMR.
• Initiate administrative tasks including onboarding providers and creating monthly schedules.
• Serve as a liaison between Solomon Medical’s billing company and facilities for insurance-related inquiries.
• Liaise between facility admins, nursing staff, and Solomon Medical clinicians to ensure smooth operations.
• Obtain reports including DME, insurance verification, prior authorizations, labs, and radiology for provider review.
• For new admissions, obtain discharge summaries and create new patient encounters within the EMR.
• Transcribe key discharge summary information (hospital course, updated meds, allergies, PMH, PSH, social history).
• Maintain professionalism, HIPAA adherence, and proper patient information handling.
