Solomon Medical in Santa Rosa, CA is seeking a full-time Behavioral Health Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our post-acute care team serving skilled nursing facilities across Central and Northern California.

This onsite role emphasizes autonomy within a physician-led model, close collaboration with facility teams, and patient-centered care for residents with psychiatric, behavioral and cognitive needs.

Qualifications

• Active California NP or PA license required
• Experience in behavioral health or geriatric care preferred
• Ability to work in a physician-led team
• Strong communication and documentation skills
• Onsite role in Santa Rosa, CA

Responsibilities

• Conduct behavioral health evaluations for residents in skilled nursing facilities.
• Prescribe and monitor psychotropic medications in accordance with regulations.
• Collaborate with nursing staff and facility leadership to update care plans.
• Ensure Medicare-compliant documentation and timely medical notes.
• Provide supportive psychotherapy and crisis intervention as needed.
