Solomon Medical Inc. is a fast-growing post-acute care company providing physician and APP services to SNFs across Central and Northern California. We are seeking a Medical Coordinator to support our Post-Acute team and ensure provider efficiency, regulatory compliance, and high-quality patient care.

The role requires strong organizational, communication, and problem-solving skills in a fast-paced, in-person environment. Bachelor’s degree preferred and SNF/healthcare experience is advantageous.

Qualifikationen

• Bachelor’s degree in healthcare administration, nursing, or a related field preferred.
• Familiarity with electronic medical record systems and medical insurance processes.
• Basic understanding of patient care, medical terminology, and HIPAA requirements.
• SNF experience or healthcare setting experience strongly preferred.

Aufgaben

• Obtain daily nursing reports regarding acute patients and prescription refill needs.
• Manage and schedule provider visits for physicians and APPs, including Medicare-aligned scheduling.
• Enter new patient demographics and insurance information into the EMR.
• Initiate administrative tasks such as facility onboarding and monthly scheduling.
• Serve as liaison between billing company and facilities for insurance inquiries.
• Coordinate with facility staff and Solomon Medical clinicians to ensure smooth operations.
• Obtain reports (DME, insurance verification, prior authorizations, labs, radiology) for review.
• Create new patient encounters and discharge summaries for new admissions.
